I’m 10 weeks post op from a nerve sparing RP. The continence is going very well but I have a problem with ED. I’ve been prescribed 100mg of Sildenafil which I take a couple of times a week. I do get a floppy result but it isn’t stiff, it also feels a bit alien, if that makes sense. Is this the way it’s going to be from now on?
 
I had my surgery Nov 21,2023 and I have the same issue. But so far they only prescribed 25 mgs….5mgs daily and 20 when I want it to get harder….but so far all I get is a bit of thickening but not hard.
I thought I was ready for that since they told me about it before the surgery but it bothers me a lot!

It takes much longer for the nerves responsible to heal. Two years is typical. Most men undergoing RP have significant issues post surgery. Hang in there and see what recover brings. Increased dosage may provide improvement and when enough time has passed there are other medications and treatments available.

I eventually recovered mostly, although it took more than a year. Sildenafil works fairly well for me. I did lose a lot in length and girth. From research that I have done, I believe that it is important to keep blood flowing into the penis by whatever method works best for you - drugs, injections, pumps, etc. it does take the nerves a very long time to heal. My doctor concentrated on recovering continence more than recovering ED. In my opinion, one should concentrate on recovering both once sufficient healing has taken place. There are a lot of opinions on this and every person’s recovery is different. I encourage you to do a lot of research because most men are not given enough information on ED recovery and incontinence recovery. Use reputable sources like Cleveland Clinic and Mayo Hospital. I wish you the best of success and yes it is a slow process for many.
 
Almost three years out and mild erections without any pill form meds which do not show any improvement. Trimix was painful but bimix works pretty well. Just difficult to make yourself jab a needle in that area.
 
Plenty enough for the purpose. It isn’t like a natural erection at the very tip but it works. I would definitely recommend trying them out. There is another alternative as well where you insert a small amount of medication into the urethra. I’ve never tried that method.
 
I had nerve saving surgery in January 2021 when I was 72. I was prescribed 5 mg of Tadalafil to take one pill daily. I had my first unexpected spontaneous erection about 2 months after surgery. I started to use a medical-type penis pump once daily after 3 months to help with the flow of blood to the penis which is important. Since that time I've had no issues with having spontaneous erections.

I'm sharing this information with you because you should not get depressed over it and give your body a chance to heal. Everyone's healing process is different. If you were sexually active prior to surgery you should be in good shape to recover.
 
I had surgery that was intended to be nerve sparing in July 2022. I think it is probably impractical for surgery to be nerve sparing when the cancer is locally advanced. I had already had to use Sildenafil for several years before this surgery. I was advised by my surgeon to work with a vacuum pump once I was continent and had waited three months post surgery. It took me about six weeks to get proficient and i had a lot of helpful advice from the pump supplier and an Australian neurologist who does emails under the name Victoria-A touchy subject.

I have always had to use a ring (sometimes two) with the vacuum pump but it does make intercourse work. I am so grateful for the pump which means that as a man with no natural erection I can function as if I did. Obviously no spontaneity, but then that was always difficult. It is a cheap reliable solution for me, but obviously every man is different and there are other options. I like not having to use medication.
 
Yes everyone is different. I had very good natural erections before the surgery. Because I was nerve sparing on one side the hospital prescribed tadalafil for six months. I didn't have any significant improvement. I started using a vacuum pump and thankfully I have not experienced any issues with loss of length or retraction. However, despite some occasional stirrings I don't have anything near a sufficient erection for love making. So we use the pump with constriction ring and we are happy with that. I am just over two years post op.
 
@Dewibach1 I would give it some time before going to Trimix. I understand it isn’t a permanent fix and will eventually stop working. Like most have recommended, give it time. Focus on healing. You had a major change to your anatomy with prostate removal. Your urologist should have told you to keep blood flowing. Sildenafil will do that, as well as regularly using the pump just to get blood in and avoiding atrophy. Good luck!
 
A large scale research project on recovery of erections found the probability to be 35%. That figure has not improved over the last decade. Men should be made aware of this and the appropriate counselling and emotional support put in place. I have come to terms with the loss and am happy with the current experience that my wife and I enjoy. Yes the VP is necessary but the love and intimacy are still the same.
 
My surgery was in late November of 2023. I was given low-dose cialis to be taken daily. I was also prescribed regular-strength Viagra to be taken twice a week. So far no results. I have been using the pump daily which works but doesn't last. The rings are too uncomfortable. I'm grateful I have a loving wife who has been patient and supportive. One question I have is: I tried Viagra even before my surgery and it never worked for me. Cialis never worked either??? Have any of you not had any success with Viagra or Cialis before your surgery??? I'll keep taking my low-dose Cialis and regular-strength Viagra, use the Pump, and hope one day I'll have success.
 
Cialus and Viagra did not do much for me prior to surgery and does not do much after 7 months since surgery.
Meeting with a men’s health clinic doctor in August but last time he suggested I might want to try the needle. People I have talked to don’t seem to mind it too much so I might try it…..
Even at 72yrs Prior to surgery my erections were good and strong and now it’s just a medium marshmallow!!!!
We will see what I decide!!!
 
I have been using injections for about two years. A little bit to overcome mentally but works well for me and definitely worth it. My two cents.
 
Just over two years post op and nerve sparing on one side apparently. Took Tadalafil for 6 months with no real impact. Use the VED which works fine and helps maintain an intimate relationship with my wife. Forty years married on July 17. You have just got to do what you need to do recognising it might not be perfect. You adjust to the new circumstances and make the best of it
