So 4months post op now. DR. wants me to start penile rehab. Daily Cialis with Viagra for meds. And use of a vacum pump and retention ring. All for gaining and maintaining erection.

My questions has anyone else had experience with a similar protocol. Was it successful? And how do you determine which pump to buy.

For those who have been able to have intercourse since there surgery. Besides a DRY SHOT should i expect anything else?
 
I am 67yo and three months post-op and still having significant bladder control issues - so haven’t bothered too much about erectile dysfunction. Am taking Cialis every 2nd day (daily Cialis gives me reflux) but haven’t bothered with pumps etc. One thing to be aware of is potential for climacturia - passing urine when you have an orgasm - not an attractive proposition for the recipient! I guess you would do a solo performance as a test.
 
I am almost 10 months post op. I have been taking viagra (25mg) daily. I also use a vacuum pump in the shower. There is a desire with these things and I can reach a dry orgasm but no hard erection. What works for me are Trimix injections. I know, sounds terrible, but I give them to myself at least twice per week. Injections are not as barbaric as they sound. Erection lasts about 30-40 minutes. I have had no problem with urine leakage as long as I empty before sex. However I am hopeful I will regain erections only using pills at some future date.
 
My experience pretty much mirrors palarino1, except I am 13 months out and have not yet tried the tri-mix. Have taken 5mg tadalafil daily since catheter removal, 20mg when attempting sex and have used a VED with limited success and had leakage . Going back to the doctor at the end of January with several questions and 58 years old. Had a highly respected/recommended surgeon and robotic assisted surgery. 🤷‍♂️
 
First, Thank you for starting this thread! I have been researching online how to orgasm post radical prostatectomy and there is precious little practical information. I will summarize what I have read and request/encourage others to add their thoughts. Please correct anything you think is incorrect.

Prior to surgery, urologist told me that he would not be able to spare the nerves that control erections due to the size and location of the tumor. At that point, I rationalized that living was more important than my sex life so I decided to proceeds with surgery rather than radiation only. I am 66 and 3 months post surgery. I am not able to have an erection. When I jerk off, I can achieve a very weak dry unsatisfying orgasm. The first time I tried, I had urine leakage. After the first trial, I have not leaked.

My philosophy is that I had a very active sex live and it is time adjust to a life more focused on other things. And I am okay with that. However, I want to rehabilitate my penis to at least restore its shape and fullness so that at least I have a full soft penis and if nature allows erections in the future, that would be welcome but I am not going to set my expectations to have an active sex life. However, for those that want an active sex life, sex is or course more than penetration so it should not prevent you from being sexually active with your partner.

This is what I have read online:
1. ED is resolved in some people so rehab is worth the effort.
2. Amazon sells penis pumps, so read the reviews prior to deciding what to buy.
3. The sooner rehab starts the quicker the recovery, whether full or partial recovery.
4. Surgery does not impact the nerves that allow orgasm. Only the nerves that allow erection.
5. Ejaculation is not possible. The semen in the testicles simply gets reabsorbed into the body.
6. Try viagra/cialis in combination with a good cock ring.
7. Massage the penis regularly to encourage blood flow and nerve repair. I do it in the bathtub.
 
Just had my 3 month post prostatectomy checkup and was ready to start pumping. Unfortunately, still using 2/3 depends a day so dr says that should clear up first. Dry at night at least. He still ordered me a pump for when I’m ready, and I’ve been taking 20 mg viagra daily. Must say I still have a helluva orgasm without the boner, so that was a pleasant surprise lol.
 
Thank you all. I've done the personal hand job myself able to achieve the organizational sensation. Just still not getting size yet. I've swallowed my pride and accepted that it's probably gonna be "Mr. Pinky" for a while (her name for vibrator) I'm ok with it.
Little concerned about the side effects of the drugs. Suffer enough congestion without encouraging it.
Finally getting pretty good control of the leaking so ready for next step. If drugs and pump don't work then will look into injections.

Updated January 13, 2022; Prostatectomy Sept 2020; Male Urethral Sling Surgery Sept 2021

After your prostate is removed, there is no semen produced. The prostate and seminal vessels are removed. The tubes from the testis are cut and closed. A very very few men have some fluid from Cowper gland (bulbourethral gland) at end of penis. You will have dry orgasms.

Penile rehabilitation is needed to stretch the penis and allow blood to flow into the penis while the nerves have time to heal. Using a pump helps restore some of the lost length.

My steps were
Week 0 - prostatectomy

Week 1 - Catheter removed; started tadalafil (10 mg) daily to help blood flow in pelvic area - not so much for erections at this point. Most men start at 5 mg. I purchased using GoodRx at a local grocery store pharmacy because my insurance did not cover it and the chain pharmacies were much higher. I pay $40 for 90 tablets (3 month supply)

Week 8 - Urologist recommended Bathmate Penis Pump - I told urologist that I seemed to be 1.5 inches shorter

Week 10 - started to see some 40 to 50% erections

Week 26 (6 months) - started Trimix penis injections (I would pump then inject so that the tissue is stretched.

Month 9 - having a few erections without shots

Month 12 - having erection about 90% pre-op

Month 15 - stopped using Trimix, Continue on 10 mg daily tadalafil (cialis) with weekly additional 20 mg tadalafil.

I purchase both the Bathmate Xtreme 5 and the Vacurect pumps. I prefer the Bathmate to the Vacurect.

Vacuum Pumps provide two functions 1) get fresh blood in the penis and 2) getting blood and holding it for pleasure. Important to note that both of the following pumps manufacturers recommend only pumping 10 to 15 minutes a day. The Vacurect STRONGLY notes to only use a tension ring for 30 minutes to keep from causing damage.

I am using the Bathmate hydro extreme 5 (the extreme has a hand pump and hydro means it uses water instead of air). I sit on the corner of the tub with cups of warm water to refill the pump. I pump 3 minutes with 2 minutes massage and repeating 2 more times for a maximum total of 15 minutes. I use a conditioning shampoo when massaging. I did not purchase the overpriced special lubricates. (Not my ideal, someone else mentioned and I like it).

Bathmate is located in the UK and I received mine in a week.

The Vacurect is more for creating an erection and using it. The Vacurect has tension rings that attach to the bottom of the cylinder. I bought the deluxe with 10 tension rings, but the Vacurect OTC with 5 tension rings is cheaper.

I am 66yo, almost 5 months post surgery now and rarely leaking at night and getting through 99% of days with only one pad and my leakage when physically active is still improving steadily. My urologist was able to spare the nerves on one side during surgery and at the one month post surgery consult told me I had a 60 - 70% chance of regaining natural erections over time. He also put me on a 5mg daily dose of Tadalafil from that point "to maintain the blood flow and prevent the penis from shrinking" ~ and it has done that, no complaints about that, my penis is consistently engorged and has not shrunk at all. Over the last few weeks since my leakage started improving steadily I have attempted to gain an erection a few times through masturbation without success ~ like others have mentioned I can achieve a mild unsatisfying orgasm like sensation but no life in the old boy yet.

My next urologist appointment is 22 March at 7 months post surgery where he said he plans to discuss and start addressing my ED, probably by increasing dosage of Tadalafil and introducing a pump and/or injections. Looking forward to this appointment and in the meantime I will keep experimenting with manual and visual stimulation.
 
@DouginOz Almost exactly my experience, Doug. I'm also 5 months out from surgery, and there is still no stirrings in my little partner. Instead of tadalafil, I am taking Viagra 20 mg a few times a week. I could not tolerate the side effects from tadalafil. I also had the Tri-Mix injection, and ended up in the ER with priapism. Very painful. My urologist suggested Bi-Mix, but I decided to hold off for now to see if my erection will start to come back on its own. I'm encouraged by the vast improvement I have made in my urinary incontinence over this period of time. Will keep you posted.
 
@JoelCairo What side-effects did you experience from Tadalafil? I haven't noticed any but am on a low daily 5mg dose at this stage.
 
I am on 5mg daily as well. If anything I have some flushing from time to time. Kinda feels on my face and neck as it does with windburn if you know what I am referring to.
 
@DouginOz I had blinding headaches. I went to bed with them, and woke up with them. I also had a lot of nasal congestion. And, I experienced acid reflux for the first time in my life with tadalafil. Now I know what so many people struggle with. Low-dose Viagra does not cause side effects, but when I try taking it in higher doses (60+ mg), I get the same side effects as with Cialis.
 
Prior to surgery I tried all three ED medications. Levitra provided the best results with significantly milder side effects.

I have resolved to start dating from sites for the asexual. I have so much dread about the pain and blood that is now always a part of sex for me, that it ruins any potential for romance. So I’ve decided to find a man who doesn’t want to / can’t have sex. I’m sick of being alone. The flu I have has been so bad that I can’t even wash my own hair. I also hit a turning point when I broke my foot recently and was non-weight-bearing for months, alone. It’s nice to hear there’s a man who can appreciate this concept of sort of giving up on sex.

As a heterosexual female, please, if you’re a heterosexual male, DO NOT, under any circumstances, pee inside a woman. You would definitely give her a severe UTI. Just one drop can be that toxic to us.

The men I’ve been with who have taken Viagra hate hated it because of the headaches, racing heart, and inability to sleep afterward. Their faces also got bright red in a sort of scary way.
