I'll echo what @Eden98 said. I didn't have a lot of spasms or leakage, but it does happen, and a pad is helpful. Besides, it keeps any lubricant from staining your clothes.
I don't know how much sterility matters,
though cleanliness is important. The catheter ends up colonized with bacteria very quickly, but I doubt introducing more is something you want to do. I'd suggest asking your doctor that question.
