Urgency after using restroom

I've been having severe urgency immediately after I pee, which is a new problem for me.

A but of background: I've had problems for many years with pain and urgent spasms as my bladder fills, which has been variously diagnosed as interstitial cystitis and OAB. I get Botox injections every few months to help control this, and after those treatments I'll usually have enough retention that I'll have to self cath. My last botox treatment was about three months ago, and I did not end up having to cath after that one.

With this new problem, I'll pee, and after I finish I'll have a strong feeling of having to pee again. If I do, very little comes out, and even that doesn't relieve the feeling of being to pee. I've cathed to see if I'm having retention, and there will be just a few drops in my bladder.

I've messaged my urologist, but my old doc left recently and the new guy they've switched me to is booked for months. I've got an appointment in December, which isn't very helpful right now.

I'll also mention that I have no UTI symptoms, other than my usual bladder pain from interstitial cystitis, and of course this new problem.

Any thoughts? I really hate new symptoms!
 
Is this mostly at night ? Or all the time ?
I have/had urge incontinence at night until I started Gemtesa
I know it’s really pricey - I started 3 days ago and it works really well !
I got 30 pills for like $480 - Walgreens was like $407
I figured it was worth the shot and it’s been working - I got up twice last night to pee and peed a lot each time n went right back to sleep.
Not sure if ur on Medicare but I got an appt at mayo today in Rochester MN 11-13 - I think my out of pocket being on Medicare is like $5 - of coarse I’ll have to drive there and overnight as I live around Chicago
 
I'm not on Medicare, but I have pretty good insurance. I met my out of pocket maximum every year, so after that I pay nothing for most of my medical care.

This is an issue that happens pretty much all the time. It seems slightly worse in the morning.
 
My “urge” started bout 5 weeks after my RP and continues.
Mine started bout 3:00pm till bout 5:00pm - crazy
Than at night which was brutal just laying in bed n every 10 minutes getting the urge but it has calmed down since Gemtesa - I’m only 3 pills in but the first 2 nights got up only twice and peed a lot each time as I said before
If ur doc could prescribe it what u got to lose ?
I talked to my doc on mychart n he just prescribed it as Oxybutinin just was two weird for me
 
Dr bales do you mind if I start this new OAB medication?
Seems safer for me

Gemtesa has not been noted to have a potential for hypertension like Myrbetriq, so it may be a better option for those who have experienced or are concerned about this side effect with Myrbetriq. It may also avoid the risk of possible side effects such as dizziness and hallucinations associated with Myrbetriq.

I also have high blood pressure so makes sense for me

I have this too and was the main reason to get botox and pfpt.

Things that I have observed:

- highly concentrated urine irritates my bladder even after emptying.
- increased bladder activity correlates to bladder aches.
- tight clothing and being on my feet for a long time and hard surfaces seems to cause more discomfort.

What helps:

- taking a break several times a day to stretch pelvic floor muscles.
- reducing stress, practice breathing and mindfulness.
- drink more water. Duh, right?
- stop fighting the bladder spasms to maintain continence. Try to calm your bladder, try to make it to the toilet but, don’t beat up your pelvic muscles trying to get to the toilet. Specially with OAB, your PF never gets a break.
 
Averaging years ago, my old urologist put a Foley catheter in me for about six weeks just to give my pelvic floor muscles a break. It helped, but having a catheter for weeks wasn't at all pleasant. I self cath, so my urethra doesn't get as irritated as it might otherwise, but it's still unconfirmed when it sticks to my urethral opening. Ouch!

I don't see my urologist until December, but I'll ask about pelvic floor physical therapy and different meds then.
