Is the Northshore MegaMax overkill?

So I'm 30 and have a neurogenic bladder. At least that's what my urologist believes I have. My symptoms started shortly after I injured my back in the military. My MRI showed spondylolisthesis and a bulging disc. The neurologist said the MRI findings didn't "clearly explain," but given the damage and the timing, that was the diagnosis they gave me. Anyway, when it first started, I had these constant abrupt sensations of needing to pee and would leak heavily. I would often empty my entire bladder. The VA provided me with the Abena M4 brief. I hated them as they it was impossible to conceal under my clothes and even though they are a heavy duty brief, I was going through 5 or 6 a day. Sometimes b/c they were at full capacity, sometimes out of comfort. They also restricted my mobility while exercising, weren't really meant to be refastened, and caused me to get a lot of diaper rashes. Eventually, I was recommended the NorthShore Mega Max and it has been great.

However, my urologist got me on a treatment plan, 15 mg of Oxybutynin and Tens Unit on each ankle once weekly, which I've been on for a couple of years now. Since then, my symptoms have greatly improved. But, to this day, I still wear the MegaMax day and night. While I do still need a heavier tabbed brief at night, I tried the pull-on briefs for the day time, but unfortunately, they are a joke. I tried several brands and sizes but they are not reliable. For one, I'm in between sizes. The mediums are too large and don't create a tight enough seal around the leg band. The smalls are too small and rip halfway on the bottom side after a few minutes of wearing, which also affected the seal. I kept having urine leak down my leg, yet my brief could be completely dry. I also have a larger penis and genitalia, so I need a wide crotch product so that the absorbent material can catch the urine.

Anyway, after trying several products, I just decided on the MegaMax day and night. I'm a grad student and just didnt have the time and energy to keep trying new products. I do have a cloth brief that works okay that I wear occasionally during he day, but it's certainly not perfect. The MegaMax is not too bothersome. Even given the fact that it is a bulkier brief, It's still hardly noticeable under my clothes, and I don't wear very baggy clothes, so that's really nice. I think it's b/c the padding is more consistent and doesn't bulge up in some areas. It's also refastenable, so I can use the toilet as needed, which is important for me given my improvement. I will say they can be kind of noisy to unstrap, but I can pee at the stall w/o even removing the straps when necessary, which is nice. They also keep my skin pretty dry even when the brief is wet, so I don't use as much antifungal cream anymore. And, they don't restrict my mobility as much.


The main thing I want to know is if anyone has a similar experience and a recommendation. One of the biggest ways my treatment has improved my quality of life is being able to leave the house for the entire day w/o needing to pack a change in my backpack, and even if I found a pull-up brief that actually worked, I definitely don't think I would make it through the day without needing to change. The MegaMax offers me that freedom. But at the same time, it just seems like overkill during the day.

Lastly, the VA provides my briefs, and they are overall a great product, so maybe I should just be thankful and let it go, but I would love to hear your thoughts and recommendations.
 
You might give incontol be dry line a chance. I wear them and they are pretty absorbant too and I also use the mega max too. If you have soaked a mega max at night give be dry night a try
 
@Ngleason Thanks for your service. Think of the Megamax as a 16 round capacity pistol rather than only 7 or 8 rounds. Great if you need it. I have mentioned in other threads using the GoSupreme pull-up for day and you could add the Trifecta water proof cover. Harder to change but less overkill and save the Mega’s for overnight.

One of these days I want to do a size analysis of the variability of products but complicated. Just like many clothing sizes are inconsistent between manufacturers as well as shoe sizes.
 
I just saw that Northshore has come out with a new diaper that is a little less bulky, and perhaps better for daytime wear. You might check it out on their website.
 
Thanks @Jdrake64850 and @DWLCPAJD for the responses! The Incontrol seems very similar to the MegaMax, which puts me in the same boat.

As far as the GoSupremes go, NorthShore sent me a sample in a small one-time with my MegaMax order. They were great, so I asked my PCP at the VA if he could order them, but unfortunately, the VA can't provide them, which is a bit odd since they supply me with the MegaMAx. Anyway, I decided to order them on my own, but when I got them, they did not work like they did before. They tore on the sides like the other brands did after about an hour. I also have a waterproof cover. It's not a trifecta one, but the material on mine is not very comfortable, so I don't use it as often.

Lastly, I just want to make clear that NorthShore is a great company with a wonderful customer service team, and the GoSupreme not working for me is not a reflection on them as a company.
 
I am over twice your age but also a veteran - British - and served alongside US troops back in the 90s. I am glad you have found a product that works for you. That is the most important thing when it comes to managing bladder problems as they give you the confidence to lead a normal life. I was left incontinent after a spinal injury and have been wearing adult nappies - in my case the Tena Slip Maxi Active Fit - for many years now. I have found them to be the most absorbent, discreet and reliable products available to me in the UK. I am lucky to have them supplied FOC through our National Health Service. They enable me to lead a full active working and personal life. I never found pull ups capable of dealing with the extent of my bladder leakage. And they are a pain to change compared with taped adult nappies. Good luck and thank you too for your service and I hope you get the support you need through your VA.
 
Thanks @adfw61 and thank your service too. I supposed even if the pull-ups worked out, they could be short-lived and potentially a tease. From what I hear, neurogenic bladders often get worse. I'm really hoping my treatment holds up. I already have a "marked" compression at my L5 nerve roots, but I guess that means there is room for it to get worse, and likely, my symptoms.
 
I would say to just keep doing what you are doing. I wore the mega max for a long time. I found I could easily pull them down enough to use the bathroom if necessary without untaping them. The best part of them is that they don't really clump up in areas with the padding. I've been trying for years to get the VA to provide me with the Abena M4 as all the other options they have are almost complete trash and barely worth the effort to wear. Those would probably be the only option you could find that will last all day without needing a change, that won't be insanely uncomfortable after just 6-8 hours.
 
Thank you for your service Ngleason. Like you, I hurt my back in the military by running all over the place in combat boats and all it took was one wrong step to injure my back. I’ve been incontinent for over two years and having to put up with the VA doctors telling you that your back is fine and there is nothing they could do for you, so I hear your frustration. I’m finally get the care I need as I’m going to see an orthopedic this Wednesday, it’s a big relief as someone is finally listening to me. Seeing a spine specialist is a little scary, but I’m tired of being in pain all the time.

To Answer your question, yes I think the MegaMax are in fact an overkill. I wear the Tena Classic/Ultra briefs as they are relatively thin. The MegaMax do offer supreme protection, but who likes walking around in a bulky diaper that causes you to walk differently. That’s just my opinion though.
 
Thanks for the response @muchardj and thanks for your service too. Is the Tena you are referring to a pull-on or tabbed brief? And, is it plastic or cloth backed? I tried some type of Tena from the VA a while back. The tena I had was a velcro-like tabbed clothe-back brief. It was very uncomfortable b/c the velcro straps would start coming undone and would rub against my leg and make me super itchy. I googled Ultra, and it looks like the one I tried. Also, I'd love to message you if you're up for it. I'm curious about what VA-approved treatments you have tried. I wanted to try Myrabetriq, but the VA did not approve it.
 
Anytime Ngleason. It’s been a rough two years for me, but I think I have it figured out now. The Tena briefs are the velcro strapped and I like them because they flex and stay in place.

The VA is trash, but free, so I guess you get what you pay for. As far as tests, had a head and back scan and it was almost identical to what your test shown. What you should do is try to see an orthopedic that knows more about back injuries. They took two X Rays and they said that your back is not OK. I hope that you are doing OK and hopefully not in too much pain. Keep me updated and would love to talk to you more as well. @Ngleason I'm curious what state you are in for the VA? I have been trying to get my VA to supply the Abena M4 for awhile now and keep getting stone walled. The best I have managed from them is the Attends Premier and they are only marginally better than the depends and tranquility. I especially hate them cause they have the velcro tabs and they loosen and irritate my thighs after awhile just like you were describing. I got my urinary issues from injuries I sustained in the army as well. It has been an absolute pain to get the army and now the VA to really deal with the issue.
 
I understand your troubles and it’s frustrating. I live in upstate NY and trying to get the VA to supply you with incontinence products is like pulling teeth. At first they gave me lite pull up. I told them that I have complete bladder loss and I need better protection. They eventually gave me tranquility briefs which were OK, but not ideal for me. It’s funny (well not really) that we both sustained our injuries from the military. I never told anybody that I actually hurt my back because the last thing that I wanted was for them to discharge me for destruction of government property. So I got out of the Navy honorable and now I’m dealing with all the issues now. I would say keep trying to see what products work best for you and don’t give up. I’ve been incont for a little over 2 years and I just found what products work best for me.
 
I'm in TN and it's been an absolute pain to get the Pharmacy to approve anything non-formulary for incontinence supplies. It's mainly a pain because they want me to try EVERY kind of incontinence product they have in their formulary which makes no sense. If I'm already using the most absorbent briefs you have in formulary (aka depends with tabs and tranquility) then none of the other stuff is gonna work. Especially not the pads or the belted underwear or stuff like that.
Thankfully (or not depending on how wanna look at it) I started to be treated for my issues while I was still in the army. After TBI I had no feeling of fullness in my bladder so didn't know when I needed to go. Then when the bladder would get to full the sphincter would finally give way and I would just end up completely flooding my clothes. Went to urology, and did some tests, but nothing was found before my ETS date and unfortunately I was deemed undeployable for the issue (understandable) and was forced to ETS 4 months early due to Obama signing the involuntary early separation stuff into effect. So I had to finish doing tests and getting diagnosed once I got out and went to the VA. Will say having to wear diapers in a Combat MOS (combat engineer) was nerve wracking just around my company/Battalion stateside. It definitely would have been mortifying having to wear them while deployed with the absolute lack of privacy there.
 
I’m so sorry your having to deal with all this draconic as TBI can be very difficult. Just remember that none of this is your fault and I pray that you are getting the care that you deserve. Keep talking and keeping up with the nurses at the VA to get the incontinence products that you deserve. Call them everyday and let them know that those products are not working and I need better protection. I remember them saying to me that they didn’t want to send me diapers because they cause skin irritation, instead they wanted to send me condom catheters. I was like, I’m not going to wear a condom all day and night. I know it’s frustrating and don’t give up pale.
 
@Draconic @muchardj Yeah, I had touble getting the NorthShore approved for the longest time. I would get the same responses about the formulary policies. I live in California and didn't get anywhere with the VA in SoCal, but when I moved to NorCal my PCP made it happen. I think I'm going to stick with the MegaMax day and night for now. Because even with the improvement and even if I found agood lighter brief, I still have days where my symptoms are worse and they're unpredictable. The NorthShore ensure I'm always protected.

@Ngleason all good. Totally understand not wanting your actual name on display. Though just so you know only other members of the site can see the posts/threads/profiles on the site.

I would definitely love to message you and talk about the process you went through to get the non-formulary stuff approved. Especially in regards to Northshore. I used the mega max for a long time but it just got to where I really couldn't afford it. Would help alot with my mental health and ability to leave the house for extended periods if I could get them to work with me on better supplies.
