Best Incontinence Product for Male side-sleeper with heavy absorbency need?

Hello,

I have been experiencing incontinence since my prostatectomy 5 weeks ago. I am in my mid-40s and do not receive care assistance (I take care of myself). After discontinuing use of the foley catheter, I used Depend products. During the day I used Depend Fresh Protection Maximum Absorbency and at night used Depend Fresh Protection with Night Defense. Neither of these products had sufficient absorbency to meet my needs. DFP lasted 2-4 hours during the day and DFRND lasted 3-5 hours tops at night before leaking down one or both my legs if I hadn’t changed underwear before then. I switched to Seni Active Super Plus. I found that this product absorbs well when primarily sitting, standing, and walking (up to 6 hours during the day and up to 8 hours overnight). I encounter problems when trying to get up from lying down in bed, sometimes after sleeping overnight, but more often after only short periods. I find when I put on a new pair of SASP underwear and lie down for 1-2 hours and then get up, I often experience leaking down one or both my legs. The problem is not that the absorbency is over capacity because the underwear is relatively light weight (the padded part hasn’t absorbed much) when I change after this happens. I think the problem is that because I sleep on my side, the urine collects between the padded area of the underwear and the cut out for the leg(s) which is not the part of the underwear designed to absorb the urine. When I toss and turn for several hours overnight, I think the urine eventually moves to the padded part meant to absorb it and I usually don’t have an issue if the capacity for absorbency hasn’t been reached. I should add that unfortunately I rarely can fall asleep on my back and/or stomach.

Any suggestions for products that work well for male side-sleepers with heavy absorbency needs? I don’t know if there are any products that specifically address the issue I described. I should add that I tried both medium and large sizes of Seni Active Super Plus. I found medium a bit snug and prefer the fit of the large. The large size stays in place, but perhaps it isn’t snug enough, leading to the frequency of the issue I described? I don’t want to try every incontinence product under the sun in hopes of finding a product that will remedy this issue. But perhaps because of the particular shape and size of my body, there isn’t an easy way to know what products would stop this problem from occurring without trying them. While the issue I described isn’t necessarily due to absorbency capacity of the pads in the underwear (I'm guessing it is a design issue?), I do still need to use underwear that has heavy absorbency capacity. Thanks in advance for your input.

As a lifelong bedwetter and primarily side sleeper there is only one fairly reliable solution. None of the disposable briefs work well for this need. I use layered thick pin on cloth diapers with good covered /fitting plastic pants. Adult prefold for overnight use that has maybe 8 layers in the middle section. Rectangular, not contoured. I then use 3 or four baby prefolds layered inside. I have always set numerous times a night and this protection combination is the most reliable method I’ve found.
Quality disposables are great for daytime use.
Good luck!

treefrog88 - So sorry you have to deal with this, hoping it will get better for you. In the meantime you have come to the right place! I have been in your place with heavy to total leaking, lets see if we can help you avoid the trial and error. Many other will have advice and hear is mine:

Northshore Mega Max are GREAT and good for Max absorbency. Start there and work back in absorbency as needed. I just tried their AirLock Brief and they are excellent as well. They will give free samples, try different sizes. They make lighter versions too if/when you need less absornbency. I typically prefer to have max absorbency and avoid leaks and have extra time for changes. I would recommend this approach especially at night, I know a full plastic diaper is not ideal but leaking is not fun either.

Fit is everything. Try other brands, BetterDry is excellent as well. I also just started using Rearz Mega Inspire that are truly amazing if you need MAX absorbency quite a bit more than Meagamax and softer more comfortable inner lining.
Make sure you are creating a "pocket" for things to flow and collect when putting things on, it helps avoid the issue you mention. Also diapers work best if you "fluff" them lightly, I just gently move/massage helps break up the fluff and absorbents and they work better. I wear Gary Wear active brief PUL cover and and lycra pant/boxer over that. This is near leak proof. Also take care of your skin, I recommend a cream and or powder. I like Caldesene

I have opted for max protection and I do not leak, I recommend the above for night and you can try different things for the day. Feel free to ask questions , happy to help. Wish you the best. Hang in there

@treefrog88 I am 3 1/2 years since my prostatectomy. It has been a long road. The best solution for me after trying everything else has been condom catheters. I was never able to sleep for more than an hour and sometimes had to change several times a night. I came across external catheters on my own. My urologist never suggested them or PT. Just doing kegels on your own is not very effective. I good pelvic floor specialist physical therapist will help you get the best results you are capable of. The external condom catheters take time to figure out the right size and method of applying them, but there are no more wet depends or awkward situations. Going out to eat and traveling become possible again. Feel free to message me if you have any questions. Good luck!

This is excellent advice!
Side sleeping is most prone to leaks. If I end up on my side I can make a megamax or equivalent leak but the PUL pants over it hold it in without getting sheets wet 95% of time.

I have little to no control over my bladder. Had been like this from a young age. The best products I have found have beef north shore mega max or xp5000 diapers. Pull-ups never worked for me but have been though many different brands of diapers

I am also a side sleeper and after my prostatectomy in Sept 2020 - In about December 2020 - I discovered NorthShore GoSupreme pull-on adult brief which holds about 38 ounces. My probably was that I was waking up after wetting. Contact Northshore (link below) to ask for free samples to see if they

I needed these for about 6 months, then was able to use the depends at night.

Free Samples (Shipping and Tax was $10.77 for 2 GoSupreme; 2 GoSupreme Lite; and 2 MagicSorb underpads (chucks))

NorthShore Go Supreme has more front padding than the GoFlex.

I will add my vote for condom catheters. I don't need them for sleeping, but I use them while golfing because of my post-prostatectomy stress incontinence. They work very well and if you don't move much at night, I would imagine they would work well sleeping. If you don't want that solution, have you considered setting an alarm every two hours at night to force you to go to the bathroom before you saturate the pad? You are young, so you likely haven't experienced BPH like so many of us old timers which forced us to get up to urinate often. It isn't really restful, but it may save you the problems with leakage.

I sleep any way that feels comfortable with an Ecoables overnight setup, side, back, stomach. I have not had a leak and everything is comfortable and breathable. Layers of absorbency lets yo customize for your needs and make laundry easier. I have diabetes insipidus so 40-50oz a night is not unusual.

I live in UK. We are unable to get North shore here I use Tena day and pants night. You are only 5 weeks I had same problem inially but 4 yr on I still need pads exercises I did for 4 years waste of time.
You will improve at night as I did and have no leakage with Tena but do getup 1-2 times at night with good flow. My problem is day when busy I need two Tena 5 pads. Not sure about condom do they fall off?
Good luck.

@Colinmiles If you get the proper size condom (and they provide a measuring device, and free samples to experiment with) they won't fall off. They roll on tight and have an adhesiveness to them which prevents leakage. Like I said, I golf with them which is a lot more motion than sleeping.

@Colinmiles They come in short and long lengths. It depends on how much you retract. They also come with partial glue, or glue from beginning to tip. If you retract or do not have much length the short one with full glue will adhere to your head and hold very well. Even if you do retract. The customer service for providers that your insurance company connects you with will explain everything. You can also request a specialist to work through any issues as you try different ones. You can also contact coloplast directly and ask for customer service. Glad to answer any questions you have. It took me a few months to become fully comfortable with them and have full confidence. Good luck!

Hi I would agree with everyone on Northshore mega max just be sure to have the correct size for you.

Also not sure if you are slightly awake when you are peeing but if so I found rolling onto my back did wonders at alleviating leaks. And eventually it just becomes second nature.

I finally chose to just wet my diaper overnight as opposed to getting up 3-5 -8 times a night. My problems are caused by my multiple sclerosis. lol I am able to walk don’t care I have to wear diapers.

Cheers

Jason

Wow, great to see so many responses and so many suggested options! I probably wasn’t aware of 95% of them. Time to do some homework and decide what I want to start with. I definitely came to the right place!

Please keep in mind that fitment is critical when trying to prevent leaking when side sleeping. Once you point your penis down whatever you wear needs to be tight enough to keep it in place.
I'm primarily a side sleeper, the only times I've woke up to a wet bed is when I get erect during my sleep.
Your doctor(s) will usually provide you with different samples to try if you request them. Try what best fits your body type and with enough trial and error you'll find what works.

I also am a tosser & turner at night. Usually sleep on my side or stomach.
I often switch between using a disposable and cloth style diapers. Simply for comfort, absorbency or economical reasons.
I find the cloth diapers to be highly absorbent and dependable for preventing leaks, plus a money saver over disposals.

Sounds like a good fitting cloth diaper and plastic pants is a great solution you might want to experiment with.

I use Dependeco and EcoAble brands for cloth diaper. No pins needed. (See profile for example)
For disposable, I like Betterdry and Seni Super Quattro, Megamax as well as some others.

Find the right brand, size and fit. Good luck.
