After going pad free (12months post op RP) my leakage has increased?

Had a fairly scripted Robotic RP. Started exercises under PT guidance for 3 months. Leakage gradually reduced and then went to only needing a thin shield which remained dry most of the time. Nothing at night with no leakage at all. I decided to try and go protection free just after Christmas at the 12 month mark. I started getting what I describe as ‘a perception of dampness’ but nothing really wet. That dampness increased to being uncomfortably very damp. I am back to the shield and have wetness in my underwear that I thought might be just sweat. I’m doing nothing to cause me to sweat?? I continue the exercises with no improvement, morning and night with some pf exercise while walking or sitting during the day. I walk at least an hour each day.

At last measure in January my PSA was 0.01.

I’m not sure what to do. Do I need a break from the exercise? Have I done too much? Do I need to have more surgery of medication. Can anyone out there provide some advice

Just curious, is there any rhyme or reason to the episodes of dampness? Are they associated with any particular time of day or any particular activity? For me, I run into trouble if I spend too much time on my feet. My suspicion is that I need a bit more pf strengthening, but I'm not as diligent with exercising as I should be.

Chuck, thanks for the answer. It is definitely more noticeable when I’m standing than sitting. Any time of day or night if I’m up. Don’t need to be doing anything strenuous and the dampness just sneeks in, no flooding at all. I even wondered if I had developed a fistula but have no idea why that would happen. My intended solution is to go back to my pt and ask for a test.

Don't want to point out what might be obvious to you already but might change of weather to cold, or spending more time outdoors in the cold be a partial explanation? Being out in the cold makes my thankfully minimal leakage worse (happened to me last night).

There is a change in weather here but not to the cold. I live in Australia and we are in the hot weather here. We are having warm to hot days. Quite dry where I live with some humid days.

Maybe the hot weather brought on the change, I don’t know.

My incontinence feels the same. I have BPH and OAB, if I don't wear a pad I feel damp all day and night.

Dust...,
There's no better life than the life you choose, which includes living it to its fullest. If you agree with that bit of wisdom, then focus on the leakage management thereafter. In that regard, you might consider joining our volunteer test group that is evaluating our external aid (see my profile) that has been researched and developed over the past five years. It is patent pending, but not yet for sale. This last iteration is what we hope to introduce to an industry player for wide distribution. Gaining positive feedback from this field testing will further that effort. Your leakage at this time may not be that big of an issue, but do keep my offer in mind. Info is available if you send an email to amhelp@comcast.net. In return, i'll forward detailed instructions that should answer most of your questions. Best wishes to you. Fynlee

Chuck11 asked me to report back after my visit to my pt. My pt is a specialist in men’s incontinence issues, highly trained and highly qualified.

I am passing on his advice as is, nothing further implied ( I’ve read some of the posts on this forum and found there are folk who seem to get offended when someone says something that they think isn’t right or doesn’t fit.)

We discussed my perception of dampness problem. He asked me about the measure of wetness I am having. I weigh my pads before and after wearing them to determine who much has leaked during the day. Even now it is in the realms of collected sweat (it is summer here and very hot).

I had done some online research and discovered that I may have leaky anal glands or haemorrhoids. So that could be the problem but that would be handled by normal doctor.

He next looked how effective my control is now, using an ultrasound to scan the area while doing kedgels. He was able to let me see the live scan. He then discussed my kedgels exercise regime which I have been doing daily. He found no problems with my control level or regime. He did suggest that every day for the exercises may causing a problem though. He said over exercise can cause the repairing nerves to become over sensitive resulting in a feeling of dampness. I now have day on and day off.

To test this theory I was to monitor myself for three days of no exercise. If the feeling went away we would know the cause. Sadly for me I saw no improvement. I will now be seeing my doc.

Interesting. I've never heard of an every other day schedule for kegels, but that does make some sense. It’s common for weight lifters to exercise a particular muscle group one day, and a different group the next. I may give the “alternating day” schedule a try myself. Thanks for taking the time to report back. Praying your upcoming doc visit is helpful!
