Penile clamp

After my RP surgery, my Physical Therapist recommended a clamp while I regained my continence. I tried both the Pacey Cuff and Weisner Clamp.

The Pacey Cuff in theory is a better design (my opinion) but it never worked for me despite many adjustments - there was always leakage. It made my penis stick outward - so it was uncomfortable if I was wearing ordinary jeans for example.

The Clamp worked a bit better, but it too was uncomfortable if I was walking/standing (the clamp would be squished when I wore jeans). I bought it off Amazon, and got a refund because it didn't work.

With both devices I just felt like my penis was being strangled....a feeling that was hard to ignore. After lots of fiddling around I gave up trying to use them.

My urologist said that if you use these devices your body won't learn how to regain continence as you recover from surgery. I know many find these useful and wear them without discomfort, but I just carried on with pads and Kegels while my continence slowly returned over 3-4 months.

I have used a penile clamp for almost 2 years now. I am 85 to 95% continent following my prostatectomy 2 years ago so I use the clamp to protect againts unwanted leaks while at meetings, retaurants, etc. I can wear for 1 to 1.5 hours without too much discomfort. They take getting use to but with the right fit they are very helpful. I use the BARD Cunningham Clamp which can be bent to better conform to ones anatomy. 100% plastic clamps cannot be bent.
 
Update October 28, 2024. I used the clamp for months 4 and 5 (Jan - Feb 2021) after RALP in Sept 2020.

I used the Lunderg Confidence Clamp for a few months - I just needed relief to get from home to doctor appointments without soaking a pad every time I got out of my car.

I prefer the Lunderg Confidence Clamp, If out of stock, try the Wiesner Incontinence Clamp ($35 on Amazon 10/28/2024).

Both Lunderg Confidence Clamp and Wiesner Incontinence Clamp have 3 sizes bottom pads with a dimple that presses on the urethra from the bottom to stop urine flow and to control the pressure on the urethral and not the whole penis. The top pad does NOT have a dimple. Both use medical grade silicone rubber to cover the bottom and top pads.

I prefer Lunderg because it has one tab that inserts into one of 4 HOLES to tighten. The Wiesner has one tab that clicks under one of 4 TABS to tighten which leaves the exposed tabs.

Both clamps documentation states to release and readjust EVERY 1-2 hours and NOT for overnight use.

I started out wearing the clamp for a few hours a day and added time before using the clamp outside of the house. I wanted to determine correct bottom pad size; pressure (4 locking positions); and how it felt sitting, standing, and walking.

Watched the pressure - I use "for Small" bottom clamp and click it in the second hole. I am able to press the head of my privates and the skin discolors and recolors appropriately. Not too tight to cut off circulation, but has been doing the job.

I have found the Weisner to work pretty well. I train in the gym every day and the clamp really helps to avoid huge leaks during the straining. My Dr and physio both recommended moderate usage as a full bladder with retention can strengthen the external sphincter. I will usually wear for the workout, then void after an hour. I still use very sparingly outside the gym, but it does help. Also good if standing at parties and gatherings and working around the house, but use sparingly
